Replacing a Lost Identification Card
|

The participating network provider uses the information on the Vision ID Card to
verify eligibility and plan coverage for the participant and eligible dependents.
If you have lost your ID card, simply click here
to download and print a new ID card. Fill out the appropriate information prior to
making an appointment or visiting your network provider.

Filing an Out of Network Claim
|
Your vision program may include an out of network benefit that allows you to
use your eye exam and / or materials benefit at a provider that does not
participate as a network provider.

instructions provided on the form to file an out of network claim. It is
your responsibility to pay the provider in full at the time services are rendered. For
reimbursement information, review your program communications or simply call
the Customer Service Center at 1-800-288-5172.
