How do I use my benefits?
Just call any of CMV's network locations to schedule an appointment. You will be asked to provide your Social Security number or health plan identification number, and your eligibility for benefits will be verified before your appointment. Once you have received your examination and/or ordered corrective materials, you will need to pay any amount not covered by your benefit program.

Where can I go to use my benefits?
If you know your plan number (from your CMV ID card), you can find the nearest provider using the Find a Provider page. If you are not sure what your plan number is, or if you have not yet received your ID card, please call member services toll-free at 800-288-5172.

Do I need to get a referral from my Primary Care Physician to use my benefits?
In most cases, you do not need a referral from your Primary Care Physician to use your CMV benefits. CMV vision benefit plans are separate from your health plan benefits. However, if you are a Medicare patient recovering from cataract surgery, you will need a referral from the doctor who performed the cataract surgery to be eligible for post-surgery eyeglasses or contact lenses.

To schedule an examination or to purchase corrective materials, simply call the CMV network location of your choice and schedule an appointment.

Can I get an eye examination from one provider and my glasses or contact lenses from another?
Yes. As long as you remain eligible for benefits, you can receive the benefits from any CMV network location, unless you are a first-time contact lens wearer. In this case, you must purchase your new contacts from your exam provider and return for one to two follow-up visits to insure that your lenses are fitted properly.

Some states do not require doctors to release your prescription for eyeglasses to you. Ask your exam provider before he/she performs the exam if he/she is willing to release your prescription.

What if I want to buy more than one pair of glasses?
Most CMV plans provide scheduled discounts for additional pairs of glasses or contact lenses. Just visit any of our network locations and you'll receive savings up to 60% off the regular retail price of your glasses or contact lenses.

What if my doctor is not in your network?
CMV encourages all members to utilize its network locations to receive their benefits. We recognize, however, that some members will choose to visit non-participating providers for their eyecare needs. Some plans have out-of-network benefits for examinations and/or corrective materials. Please contact our member services department at 800-288-5172 to see if your plan includes this feature.

Can I order my contact lenses through the mail?
Yes, if you have worn contact lenses before. Just call Contacts Direct at 800-987-LENS, and a representative will assist you with the ordering process. If you are a first-time contact lens wearer, you must purchase your new contacts from your exam provider and return for one to two follow-up visits to insure that your lenses are fitted properly.

Where can I get ID Cards?
If your benefits are provided through your employer, you can contact your Human Resources department for a replacement ID card.

If you receive your benefits through your health plan, you may not have a separate vision ID card. If you are unsure, contact your health plan's customer service department.

Can I use my benefits in conjunction with another plan or store promotion?
When you visit a CMV network location that is running an in-store promotion, you can choose to utilize your CMV benefits or the in-store promotion, whichever is preferable to you. You cannot combine your benefits with an in-store promotion.
